Upon arriving at our office for your first visit you will be asked to complete several forms, including a registration form, medical and
dental form. For your convenience, the forms can be found below so that you may print and complete the forms at home prior to your appointment.
In some cases we may ask you to sign a dental release form, so that we may obtain recently taken x-rays or notes from your previous dentist. If you require any additional information please call our office and speak to a team member....705.352.1028.
