Our office DIRECT BILLS, electronically submits, all dental claims day of treatment. We will gladly mail or fax any claims, when need be.

As a convenience to our regular patients we ACCEPT ASSIGNMENT OF BENEFITS, upon agreement. Upon acceptance, the patient has their insurance company pay us directly the percentage of the dental treatment fee that is covered by their dental plan. Any monies not paid for by your insurance company are to be paid for day of treatment, known as co-pay.  

Many plans will cover a percentage of costs for eligible services. For example a plan may cover 80 % of the cost of basic/preventive services such as examinations, fillings, and cleanings. This percentage is based on the costs outlined by the plan provider and may vary from the actual costs of the treatment. Major procedures such as crowns may be covered at 50%.

The details of your insurance plan can be obtained from your Human Resource Department or Insurance Plan Provider. If you need help with getting this information we will gladly help.

YOU ARE RESPONSIBLE FOR ANY MONIES NOT PAID BY YOUR INSURANCE COMPANY ON THE DAY OF TREATMENT
It is the responsibility of the patient / responsible party to be aware your coverage and benefits and are responsible for any fees not covered by your insurance company.

CO-PAY PAYMENTS ARE DUE ON DAY OF TREATMENT....Co-pay, deductible payments and fees for non-covered services are due at the time of service.
